Makes referrals to all units within the Department of Social Services, County government departments, community agencies and other resources. Provides long-term care information and assistance for seniors, and adults & children with disabilities. Conducts intake for referrals to Adult Protective Services and Children’s Preventive Services. </li> </ul> <ul> <li>Adult home: 845-364-3590 <br> Rockland County operates a non-profit, 41-bed facility at the Dr. Robert L. Yeager Health Center in Pomona, offering a secure, stimulating environment for adults who cannot live independently but who do not need medical or nursing care.</li> <li> Adult Protective Services: 364-2020  for referrals. <br> Crisis intervention for adults in need of immediate protection without regard to financial status.   </li> <li>Child Protective Services: 364-3512 <br> Investigates reports of child abuse and maltreatment. Staff is available 24 hours, 7 days a week. For reporting suspected abuse or neglect: Toll-free state hotline 800-342-3720; For mandated reporting{ Toll-free state hotline: 800-635-1522.   </li> <li>Child Support Enforcement Unit: 1-888-208-4485. Establishes paternity; locates absent parents; obtains, collects and enforces child support payments to temporary assistance clients and private individuals regardless of income. CSEU is located at 2 New Hempstead Road, 2nd Fl, New City and is open from 8am to 5pm Monday through Friday. To schedule an appointment, call 845-638-5917.</li> <li>Consumer Directed Personal Assistance Program (CDPAP) - CDPAP provides partial or total assistance for personal care tasks, home health aide tasks and/or skilled nursing tasks under the instruction, supervision and direction of the consumer or the consumer's designated representative. Must be Medicaid eligible.</li> <li>Housing Unit/Emergency Shelter (845) 364-3150 <br>Offers comprehensive social services to families/individuals facing potential homelessness. Coordinates with family members, landlords and attorneys to avert impending evictions, negotiate payment of rent arrears and explore housing alternatives. Emergency shelter placement is provided when necessary.</li> <li>Childcare Subsidy Program: 845-364-3797  </li> <li>Authorizes day care subsidies for working parents receiving public assistance and low-income working parents not receiving public assistance.</li> <li>Employment Services: 364-3143 <br> Assists public assistance and food stamp applicants and recipients to secure gainful employment through a variety of employment programs, approves vocational exemptions for school attendance, monitors clients in training programs, and monitors the attendance of alcohol and drug abusers in rehabilitation programs </li> <li>Family Preventive Services: 364-2020 for referrals. <br> Services without regard to income to avoid placement of children outside their homes due to family problems. Family teams are located in Nyack, Spring Valley and Haverstraw. </li> <li>Intensive Preventive Services: 364-3166 <br> Intensive, short-term, home-based services are provided when a higher level of intervention is required as an alternative to placement of children in foster care.  </li> <li>Food Stamps: 364-3100 <br> Establishes eligibility for food stamps for low-income households.</li> <li>Home Energy Assistance Program (HEAP): 364-3478<br> Administers the energy program which helps county residents with limited incomes pay their utility or fuel bills.</li> <li>Medicaid: 364-3040 <br> Establishes Medicaid eligibility for families not on public assistance, authorizes issuance of Medicaid cards to SSI clients, determines Medicaid eligibility for individuals who require long-term institutional care.</li> <li>  Special Investigations Unit: 845-364-3200 <br> Investigates all complaints regarding welfare abuse, audits cases in which a high probability of fraud exists, and attempts to recover overpayments to clients.</li> <li>Temporary Assistance: 845-364-3100 <br> Evaluates and processes applications for those who demonstrate need for financial assistance or one-time emergency aid.</li> </ul>
